**CANCELLATION POLICY**
-PLEASE BE KIND AND CANCEL YOUR APPOINTMENT AHEAD OF TIME.
We require 24 HOURS NOTICE for any cancellations due to high demand. If the client cancels less than 24 hours before the scheduled appointment they will be asked to pay 50% of the treatment cost. The client will be charged 100% of the treatment fee if they give less than 4 HOURS NOTICE OR DO NOT ARRIVE. Any clients that are LATE will be charged FULL PAYMENT of the treatment time they have booked.

**BOOKING AN APPOINTMENT**
-PLEASE CALL KING STREET CHIROPRACTIC & HEALTH CENTRE TO BOOK THROUGH OUR FULL TIME RECEPTION. (306-634-2918)
-We ask NEW PATIENTS to arrive 10-15min early to fill out a patient history form. A verbal patient history and general assessment will be included in the treatment time for client & therapist safety.

*Therapeutic will require more assessments based on the client’s needs therefore the assessments will also be included in their 60 minute treatment time
