Hometown Primary Care, PLLC is a Primary Care practice, with a Family Nurse Practitioner being the lead provider.

Our office offers a wide range of medical services, from routine visits to lab work. We also offer a Diabetes Management Program to the people of the community affected by Diabetes.

We are active in our community, offering different programs throughout the year to get our patients and members of the community active, and to promote a healthy lifestyle.
