The Agency’s mission is to provide home care services to clients in their homes, assisting them to achieve the highest level of potential in their day to day self-care activities. We are committed to providing high quality care through attentive workers who recognize the importance of comprehensive care of the client.

The Agency provides the care needed to adequately maintain patients at home. In the environment of his/her home, the patient may be cared for thoroughly by a staff of skilled professionals dedicated to maximizing the “Wellness” state of the patient.

Each patient admitted to the Agency is evaluated by a professional case manager. The plan of care established by the case manager is evaluated by the interdisciplinary team on a regular basis. The plan of care is updated to meet the patient’s changing needs.

The objectives of services provided by the Agency are to meet the physical and psychosocial needs of the patient in time of short or long term illness, disability, or emotional crisis, which may threaten the normal pattern of life. Services provided will contribute to the prevention of the disintegration of individual or family life.

The mission of the Agency is to provide services that maintain personal choice whenever it’s feasible and to maintain a way of life which allows the patient to retain as much independence and participation within the family and community as possible.
