Independent Community Living

Support is based on each participants Person Centered Plan goals and consists of any number of services including but not limited to; bathing, grooming, toileting, medication assistance, cooking and feeding, time and money management and community inclusion. All services will be provided to enrich the participant's life and ensure their ability to live in their own home and can be provided in home as well as a community setting.
