The Surry County Health and Nutrition Center provides the following services:
CHILD HEALTH SERVICES: Involves all aspects of pediatric assessment and health care for patients to 18 years of age and for those patients with Medicaid to 21 years of age.
SENIOR SERVICES: This division provides in-home care to elderly and disabled individuals wishing to remain at home, because, truly, "there's no place like home..."
FAMILY CARE COORDINATION SERVICES: This program provides case management for high-risk pregnant women and children up to 5 years of age and postpartum/newborn home visiting by an R.N.
NUTRITION/WIC/DIABETES: These services include the Supplemental Nutrition Program for Women, Infants, and Children (WIC), Breastfeeding Promotion and Support, Community Nutrition Education, Medical Nutrition Therapy, and Diabetes Self-Management Education. Our nutrition services are designed to promote healthy lifestyles for people of all ages.
HEALTH PROMOTION/HEALTH EDUCATION: This section provides health promotion and disease prevention services at schools, worksites, and the community.
COMMUNITY HEALTH SERVICES: These services include Immunizations, Adult Primary Care, Migrant Farmworker Program,
Family Planning, STD/Communicable Disease, and other services for the protection of the community's overall health.
ANIMAL CONTROL: This section helps prevent the spread of disease by assuring that the public is protected from environmental disease spread by unvaccinated, dangerous, and/or wild animals.
ENVIRONMENTAL HEALTH: The principal function of this section is to prevent disease by ensuring that no unsafe food or water is consumed.
