At AgeWell Medical Associates, we strive to provide high quality, comprehensive primary medical care to adults over the age of 60.

Under the visionary leadership and direction of Jeffrey Kulp, MD, AgeWell offers innovative patient centered care to meet the increasing demand for primary care services in this community. Our model for primary care for seniors is built on a team based approach. We utilize nurse practitioners as primary care providers to their own patients, and other healthcare professionals to make up your primary care team, while working collaboratively with Dr. Kulp.

AgeWell has embarked on the journey to become a patient centered medical home (PCMH).
We want to be a center of excellence for the older adult. The PCMH concept equips primary care practices to do their part in achieving the “triple aim” of health care reform – better patient experience, population health, and reduced costs.
