The mission of Alston Brook is to be a community rehabilitation and nursing facility; providing resident and family services with compassion and quality. The facility by design enhances the quality of life for each person receiving services at Alston Brook. For the family, a quiet satisfaction, an inner feeling of peace that comes from knowing that your loved one is receiving the very best care possible by a kind, caring staff in a new and modern facility.

Alston Brook is about people. It is designed around the resident's needs and with those needs at the focal point of all interior and exterior planning. The goal is to provide intensive rehab and healthcare services in a home like environment with units designed specifically for the services provided on the unit.

To create a home like environment, Alston Brook is divided into three different units. (Rehab, Dementia and Traditional Skilled Nursing). Each unit has its own dining, living, and recreational areas. Short corridors and living units within larger units add to the "I'm Home" feeling. Lots of windows bring in natural light and sunshine. Gardens in both interior court yards and around the facility provide natural beauty for the residents to enjoy. Porches and patios entice the residents to enjoy the fresh air and the country like setting.

Upon admission at Alston Brook, our team of professionals assess each resident's needs, and develops an appropriate plan of care to ensure that each resident functions at the highest possible level.

Under the direction of each resident's personal physician, team members contribute special knowledge and skills to design an individual plan of care for each resident.

The goal of Alston Brook is to return each resident to his or her highest level of function. Individual programs are tailored to include goals to ensure that appropriate and functional skills can be identified, learned and reinforced.

Our facility provides a variety of programs with the highest standards of quality. Here are the programs in full detail:

"LILLIAN'S WAY"

"Lillian's Way" is a specialized unit dedicated to serving resident's with dementia and memory loss. The unit is designed to provide a safe environment for residents with memory loss and dementia related care needs. The unit doors are alarmed to alert staff when residents attempt to wander from the unit. Enclosed outside areas have been designed to allow the dementia residents safe walking and restin areas around beautifully landscaped court yards, a gazebo and a screened-in porch. The nursing staff for the unit have received specialized training on care of the dementia resident and a unit coordinator is responsible for the unit. Continued training is required of the staff in order for staff to be abreast of the latest enhancements and advancements in the care of dementia residents. Thus, Alston Brook has a special focus on the care of residents residing in "Lillian's Way".

"GRANNY'S PLACE"

Sixty-four beds are dedicated to residents in need of long-term skilled nursing care. This unit is referred to as "Granny's Place" and is designed to enhance the quality of life and quality of care for residents with long-term debilitating conditions. Social services and activities personnel play an active role in assisting the long-term resident in adjusting to his/her condition and the facility. Nursing staff provide the necessary care and activites of daily living to meet the resident's needs. Restorative nursing and Therapy staff provide therapy modalities as needed or required by the resident to assist the resident in maintaining his or her highest level of functioning. Residents on this unit receive care from staff trained in provision of long-term care.

"GREENE'S COMMONS"

Resident's admitted to our short-term rehabilitation unit called "Greene's Commons" receive specialized intensive therapies and nursing services focused on assisting the resident in reaching his or her rehab and physical functional goals. A separate physical therapy department with 854 square feet of treatment space and a separate occupational/speech therapy department with 440 square feet of treatment space are located on the unit. Bot departments have the latest in equipment and advanced trained therapy staff to assist residents in regaining functional skills and to be able to return to independent living. The mission of this unit's staff is to assist the resident in returning home as soon as possible. Thus, in addition to nursing services the following therapies and modalities are available for resident's of "Greene's Commons":

REHAB SERVICES

Physical Therapy:
Our physical therapy professionals assist residents with:

Mobility:
Increasing the ability to ambulate and/or use wheelchair propulsion; improving skills to enable residents to achieve maximum independence.

Balance:
Improving resident's balance to enhance safety of transfers and/or ambulation activities.

Positioning:
Assessing lower extremity needs for orthotic devices to prevent contractures or improve mobility. Wheelchair positioning for restraint reduction.

MODALITY TREATMENTS

Electrical Stimulation, Ultrasound, and Short-Wave Diathermy treatments are available. Medical conditions which may benefit from modality-based treatments including edema management, pain control, contractures, wound care, and muscle strengthening.

LOWER EXTREMITY FUNCTION

Improving resident's range of motion, increasing strength through progressive resistance exercises or muscle re-education.

OCCUPATIONAL THERAPY
Our occupational thearapy professionals assist residents with:

Household Management Skills:
Short-term residents with a goal of returning to independent living will receive occupational therapy modalities in house hold management skills such as cooking, cleaning, storing and retrieving objects with adaptive devices. Home inspections at resident's request and prior to discharge will be conducted by a physical and occupational therapist for recommendations on adaptation of the independent living environment and to accommodate the resident's return home and enhance the resident's success in living independently.

Activites Of Daily Living:
Increasing resident's independence in self-feeding, dressing, bathing, and hygiene by learning new or compensatory techniques with or without adaptive equipment.

Positioning Techniques:
Assessing upper extremity needs for orthotic devices to prevent contractures or improve mobility. Wheelchair positioning for restraint reduction.

Functional Transfers:
Improving transfer skills for wheelchair, walker, or chair level to toilet, bed, tub, and shower.

Upper Extremity Function:
Improving resident's active range of motion, dexterity, and coordination. Muscle re-education for partial paralysis.

SPEECH PATHOLOGY
Our speech language pathology professionals assist residents with:

Speech/Voice:
Improving resident's slurred speech, hoarseness, and nasality.

Language:
Improving resident's ability to understand others and express themselves verbally, in writing, and gesturing.

Hearing:
Improving resident's ability to adapt to hearing loss and better understanding of others.

Cognitive Linguistics:
Improving cognitive skills to reduce confusion and disorientation; improving impaired problem solving and reasoning skills.

Swallowing:
Improving resident's ability to swallow and minimize choking. Assist with poor tongue and lip movement and difficulty with chewing.

REHAB SERVICES AND REHAB TEAM

Rehab Services will be offered to ALL residents of Alston Brook. For the "short stay" resident with a goal to return home, therapies and the Rehab Team will focus on intensive daily therapy seven days a week to maximize the resident's return to independence as soon as possible. For the dementia and skilled nursing care resident, therapies and the Rehab Team will focus on assisting the resident to retain his or her maximum level of functioning.
