Assisting Alaskans Care Coordination

We provide care coordination for Alaskans eligible for Medicaid waiver and TEFRA programs. These services are provided to Alaskans birth to death and are billed directly to Alaska's Medicaid program. Information and referral services are provided at no cost. We serve Alaskans statewide on a space available basis; we have offices in Anchorage and Wasilla with face to face service available on a space available basis to Alaskans on the road system.
