Choosing Health in Lake County (CHIL) is a county community coalition.
The group was formed in March of 2016.
The coalition was made possible by a grant project from the Centers for Disease Control (CDC). The project is called the National Implementation and Dissemination for Chronic Disease Prevention. The CDC awarded the National WIC Association (NWA) funding to incorporate this project on the local level. On the local level, the project is called the Community Partnership for Healthy Mothers and Children.
The purpose of the coalition is to implement population based strategies in the community to strengthen and improve the communities' health.
The focus is to reduce preventable chronic disease by improving access to healthy foods.
In addition, the coalition will work to reduce and prevent chronic disease by effecting access to disease management services.
Thus, having a positive impact on poor health outcomes for Lake County residents.

Currently, the coalition is composed of 15 members from 12 different sectors of the community. Our diverse membership includes individuals from local non-profits, public education, faith-based organizations, food pantry, university extension, local farmers, public health, local government, food advocate, community members, community outreach and more.

The coalition meets every month to discuss local programming and the implementation of its action plan in the community.
