Communicare Connections offers a wide variety of services for individuals with a diagnosis of Intellectual and Developmental Disabilities. Each service is tailored to help individuals achieve more independence, develop self-confidence, and discover avenues that lead to rich and fulfilling lives. Listed below are the services offered as well as a brief description of each:
Home/Community Supports: provides assistance with personal hygiene, meal preparation, eating, dressing, shopping, leisure activities, community integration, and light housework
Adult Day Services: assist and support individuals in retaining and/or improving skills which afford the greatest level of independence possible, provides numerous and varied opportunities to participate in activities in the community
Community Respite: provides caregivers a break from constant care giving and provides the individual with a place to go which has scheduled activities to address individuals’ preferences/requirements and to provide for the health, socialization needs, and community integration of the individual
Pre-Vocational: meaningful day activities of learning and work experiences, including volunteer work, used to develop general strengths and skills that contribute to paid employment in an integrated setting
Job Discovery: activities and exploration that lead to the development of a person-centered career profile and employment goal and career plan
Supported Employment: intensive, ongoing support to obtain and maintain and individual job in competitive or customized employment or self-employment
Behavior Support Intervention: provides assistance to individuals, staff, and family members to assist them in implementing intervention programs.
