**Dr. Jeri White, DO**

Dr. Jeri White is a Doctor of Osteopathic Medicine, or DO. What is that? A DO is a fully licensed physician who practices in all areas of medicine. They use a whole-person approach to treatment and care. In addition to the basic medical training, DOs receive additional training in the musculoskeletal system. By combining this knowledge with their training to listen and partner with their patient, they offer patients the most comprehensive care available in medicine today.

Dr. White is a board certified family physician and has been practicing medicine for 20 years. She also has a Masters of Science in Health Professions Education. She has practiced in a variety of settings including emergency care, urgent care, geriatric care, and family practice. In addition to her training as a DO, she did an additional fellowship in osteopathic manipulation allowing her to be knowledgeable and use even more techniques than the average DO to provide a variety of care options. In 2009 she struck out on her own. She currently is the medical director at a local nursing facility, where she provides primary care for most of the residents, and for the past several years was doing osteopathic manipulation in her outpatient clinic. She is getting ready to open a much needed full time family practice here in Grand Junction.

Dr. White's patients have indicated they appreciate the time, compassion, and knowledge she gives to each and every one of them. It is Dr. White's goal to partner with her patients to help achieve their optimal health. By opening a family practice she hopes to help even more individuals. We will be bringing on a Nurse Practitioner to help with meeting the health demands of our patients and allow us to serve more patients. **Charlotte Chance, FNP**

To offer more options to the community and be able to take more patients we have brought on a wonderful addition to our clinic. Did you know that Advance Practice Nurses (APN) such as a nurse practitioner are licensed registered nurses (RNs) who have completed a nationally accredited graduate APN degree, passed a national
certification examination and been recognized by their state board as an APN. They are trained to work across all populations and provide expert direct care in the form of health assessments, prevention, diagnosis and management of common acute and chronic illnesses.

APNs in Colorado are credentialed by the state Board of Nursing through inclusion in the Advanced Practice Registry. Registration as an APN in Colorado allows them to practice independently, including diagnosing, testing, treating, ordering durable medical equipment, documenting current health status and signing documents that give advance directives for end-of-life care. Colorado is among approximately 20 other states that also allow APNs to practice independently without the supervision or consultation of a physician.

Charlotte is a Board Certified Family Nurse Practitioner through the American Nurses Credentialing Center and a member of American Nurses Association. She received both her Bachelor of Science from Colorado Chrisitan University and a Masters
degree in Nursing from University of Colorado at Colorado Springs.

Prior to joining Family Values Medical Clinic, Charlotte worked as a nurse practitioner in an urgent care and a registered nurse at St Mary's Regional Medical Center. During her nearly 5 years there she worked with cardiac and cardiovascular intensive care patients.

In her free time, Charlotte enjoys horse back riding, camping, and traveling cross country to show horses. But, what she enjoys most is being with family – especially her husband and two children.

Practice Philosophy:

Charlotte believes that by treating each person as an individual, she is best able to help her patients maintain their health. She feels that the patient-provider relationship should reflect that of mutual respect and collaboration. Through this relationship, patients should be empowered to take an active role in their healthcare plans and
decisions. Charlotte feels that in partnering with her patients providing both education and support she enables them to achieve their healthcare goals.
