In Florida, there are four full-time, fully staffed offices located in Jacksonville - Oak Street, Jacksonville - Perimeter Park, Daytona Beach and Lake Mary. Other Florida offices include Clermont, Fernandina Beach, Lake City, St. Augustine, Palatka, Titusville, Orange City, Palm Coast, Kissimmee, Orlando, Mount Dora and Lady Lake. Southeast Georgia patients are provided for in our Brunswick and Waycross offices.
