Forte Residential, Inc - Forte Home Health Care, Inc.

Forte Residential, Inc is a certified Medicaid Waiver Provider in the state of Indiana. We provide services to recipients of the Community Integration and Habilitation (CIH) Waiver, Family Support Waiver (FSW), and Aged & Disabled (A&D) Waiver.

Forte Residential, Inc provides Residential Habilitation Services (RH1O, RH2O), Community Habilitation (CHIO), Respite Services, Attendant Care and Homemaker Services as allowed by each respective waiver. Forte Residential also provides Caregiver Support Services.

Forte Residential is currently approved to provide waiver services in 31 northern Indiana counties.

Forte Home Health Care provides Medicaid PA (home health aide and skilled nursing) services for individuals requiring assistance in their homes. Forte Home Health Care provides services in 21 counties in Indiana.
