APPT NOTES: On Mon - Thurs our hours are till 6:00 pm but the last appt time for established patients is at 5:15.On Wednesdays we have patient appts in the Afternoon ONLY. See above hours.
On Fridays we are open but there are NO patient appts due to
this being a surgical day.
NEW PATIENT NOTES: New pt appts start at 8:30am and the last appt time for NEW PTS ONLY is at 3:15pm.
INS NOTES: We accept MOST insurances, if you are unsure please call our office. Referrals are required by some INSURANCES, if you are unsure, call the number on the back of your insurance card. If the ins company requires a referral it is the Patient's and their PCP'S responsibility to get the referral BEFORE the appointment day. If you do not have insurance, your first appointment is $96 up front. Any other charges can be billed.
PAYMENT INFO: We accept Cash, Checks, AND NOW CREDIT/DEBIT Cards.
