We are now billing the following health insurance companies for child and adult immunizations: United Healthcare, Aetna, BlueCross/Blue Shield, Coventry, HealthLink and Humana.

*Blood Pressure Screenings- Wednesdays 1:00-3:00 p.m.
*Cholesterol and Glucose Screening- By Appointment 4th Thursday of every month
*Diabetic Screening- annually in November
*PSA screening for men- annually in September
*Family Planning Clinic- Monday-Thursdays 8:30 a.m.-4:40 p.m. and second Thursday of every month 10:00 a.m.-6:00 p.m.
*Family Case Management- Monday-Thursday 8:30 a.m.-4:30 p.m. and second Thursday of every month 10:00 a.m.- 6:00 p.m.
*Immunization Clinic for Children- 1st, 2nd, 3rd Thursday by appointment
*Pregnancy Testing- Monday-Friday 8:30 a.m.-4:30 p.m. and second Thursday every month 10:00 a.m.-6:00 p.m.
*T.B. Skin Tests, Allergy, B12 and Adult Immunizations- Tuesdays 8:30 a.m.-12:00 p.m. and 1:00 p.m.-4:00 p.m. By Appointment, walk-ins accepted
*W.I.C.- Monday- Thursday 8:30 a.m.-4:30 p.m. (by appointment) and second Thursday 1:00-6:00 p.m.
WIC is the Supplemental Nutrition Program for WOMEN, INFANTS and CHILDREN. WIC provides supplemental foods, health care referrals, and nutrition education for pregnant, breastfeeding and non breastfeeding postpartum women, and to infants and children up to age five who are found to be at nutritional risk. *Breast Feeding Counseling by a Certified Lactation Counselor- no appointment necessary
*IL Breast and Cervical Program- by appointment
*Home Health services available for homebound persons in Jersey County are Skilled Nursing, Physical Therapy, Speech Therapy, Occupational Therapy and Home Health Aide.
Physician orders are necessary for these skilled services.
