Life Circles PACE

LifeCircles Program of All-Inclusive Care for the Elderly (PACE®) offers support to seniors and their family with the goal of keeping seniors at home as long as possible, with care available 24/7. LifeCircles is a complete health care program that includes items such as preventative, specialty, restorative and primary healthcare services as well as activities, in-home care, medical transportation and much more. People who have both Medicaid and Medicare pay nothing for the program unless admitted to a nursing facility or seek medical care outside of the approved network of providers (except for emergency services).

Who can join a PACE® plan?

You can join PACE® if you meet the following conditions:
-You are 55 years old or older.
-You live in the service area of LifeCircles (Muskegon County and Ottawa County zip codes 49409,49417,49456,49448,49403,49404).
-You are certified by the State as meeting the need for nursing home level of care.
-You are able to live safely in the community when you join with the help of PACE® services.
