WHAT IS HOSPICE CARE?

Hospice is a program of quality and compassionate palliative care for people facing a life-limiting illness or injury. It consist of a specially trained team of professionals that provide overall management of illness, patient well being, direct medical care, pain management, and emotional and spiritual support based on individual's needs, wants, and desires.

Hospice is a benefit provided by Medicare and monitored closely by the medical director(s).

The hospice service can be discontinued if the patient's health improves or the condition goes into remission. The patient also has the right to stop hospice care or change providers at any time.

WHO NEEDS HOSPICE CARE?

Patients who have been diagnosed with a terminal disease or with overall decline in health status and are not expected to improve.

Example diagnoses that meet hospice criteria (but not limited to) include:
COPD (Chronic obstructive pulmonary disease)
CHF (congestive heart failure)
Heart disease
Dementia
Cancer (certain types)
Alzheimer's disease
Renal (kidney) disease
Liver disease
Stoke
Coma
ALS (Lou Gehrig's disease)

Hospice Service is as much for caregiver support as it is for patient care.

Hospice emphasizes palliative care and comfort.

Applies to patients both at home and at nursing facilities as facility staff is often not trained to be experts in palliative care.

HOSPICE STAFF INCLUDES

Physician services - with several medical directors to ensure the best quality of care

Nursing services - nurses are available 24/7 through normal business hours and a real person on call every day. No answering service!

Certified Nursing Assistants (CNA) - CNAs are available to help with personal care activities including: bathing, dressing, feeding, light housekeeping, etc. CNAs can be available on a daily bases when needed. The backbone of our hospice services.

Social services - social workers provide guidance and support for emotional and financial concerns. They provide additional services to families in need.

Spiritual service - we have a diverse group of chaplains that are trained to meet the spiritual needs of patients and families.

Volunteer services - volunteers are trained to help with patients and family members. Volunteer duties can include, companionship for patients, respite for caregiver, help with daily activities -grocery shopping, painting nails, walking, etc,- and even administrative duties in the office.

Additional service - We have partnered closely with reputable and genuine vendors outside of hospice to help ensure we give the highest quality of care. These services include (but again not limited to) physical therapy, durable medical equipment, help with placement for respite or long term care, homecare (sitter) services, etc.
