The primary care team is comprised of the primary care physicians, non-physician providers and medical assistants. Our practice focuses on the healthcare needs of adults. I am board certified in internal medicine and offer acute illnesses evaluation; chronic conditions management and preventive healthcare services. Our practice collaborates with a network of local hospitals in Worcester County, including UMass Memorial Medical Center and/or St Vincent Hospital—based on the location of the acute care hospitalization of our patients. We also collaborate with a network of surgical and nonsurgical specialties and subspecialists for the purpose of optimally addressing the healthcare needs of every patient in a scope-of-practice sensitive and specific ways. Meadowbrook is located in West Boylston, Massachusetts, specifically on the West Boylston side of the Worcester-West Boylston city line, and 100 yards from Interstate-190, Exit #4. Our office has handicap access; accessible by public transportation such as WRTA-bus along route 12 and by driving. There is plenty of free parking.
Our mission is to provide high value healthcare from an out-patient physician office practice. High value healthcare is defined as patient-centered, high quality, effective and efficient health care. Patient-centered care means that in every provider-patient encounter, the patient is put first. Put another way and in paraphrasing Institution of Medicine, patient-centered care is health care that establishes partnership among all providers involved in the healthcare process, the patient, the patient’s families or primary care giver—when appropriate to ensure that health decisions about the patient respects the patient’s wants, needs and preferences and that the patient has the necessary education involved. High quality health care involves the use of evidence-bases and/or best practice in our day-to-day health care delivery. Effectiveness refers to the health outcomes piece: the care we deliver from our primary care location will be outcome-driven and measurable. Accountability for the outcome we target will be shared among all stakeholders involved, including our patients, our primary care team, the physician specialist referral network we use, the hospital system locations where our patients receive care and payers. Efficiency refers to and outlines our utilization of business practice tools in ways that target enhanced patient experience of care; our office practice strategy conscious of cost containment and our philosophy on disease management decisions which emphasizes necessity-documentation for every healthcare management interventions we propose.
Meadowbrook accepts most insurance coverages and is currently accepting patients from Central Massachusetts, Worcester Metropolis-in particular and from locations beyond. We do not discriminate on-the basis of race, color, ethnicity, national origin, sexual orientation, gender identity, culture, or religion.
