• Management of acute and chronic illnesses and diseases:
Diagnose and treat health problems, prescribe medications, teach health promotion and disease prevention
Collaborate effectively with other healthcare providers to promote better communication, healthcare management, and positive health outcomes
Support the patient with self-management of diseases such as high blood pressure, diabetes, high cholesterol and asthma, and assist with skills to build the patient’s confidence and assist with acquisition of supplies and medications
In cases where patients do not have a primary care doctor, the clinic providers will diagnose an acute or chronic illness, get them started on medication, manage changes in medications, educate them about their treatment, and help them connect with a doctor or specialist if necessary
If a patient already has a treatment plan for a condition, such as high cholesterol, he or she can use the clinics for blood tests, and have results sent to their doctor or specialist
