ResolutionCare:

• Mission: Bringing capable and compassionate care for everyone, everywhere as life approaches completion

• Vision: Adapting care delivery to reach people where they live, and on their own terms

• ResolutionCare is a community Palliative Care team, including a Physician, Nurse Practitioner, Social Worker, Chaplain, and Community Health Worker, as well as dedicated Operations Staff.

• ResolutionCare will extend the capacity for specialty Palliative Care services in our community by replacing the Outpatient clinic and infrastructure with a home-centered model, and in rural and underserved communities in California that lack access to such services on the ground. Using only house calls, and virtual house calls that leverage ubiquitous cloud-based videoconferencing technology, our team will provide care where people live and on their own terms.

• ResolutionCare will provide education and mentorship in basic Palliative Care skills in partnership with Project ECHO at the University of New Mexico. Again, leveraging available videoconferencing technology, the ResolutionCare team shares expertise openly with case based learning over time to learning units distributed in Primary Care, Oncology, Nephrology, Hospitalist, Intensivist, and Emergency Department settings.

• ResolutionCare has developed committed partnerships with payers (e.g., Partnership Health Plan of California) and is innovating with new value-driven payment models, foundation grants. An incredibly successful Indiegogo crowdfunding campaign raised $137,770 from over 500 donors for the necessary seed capital to launch.

ResolutionCare launched January 5, 2015 with many Committed Partnerships established and many more Partnerships awaiting approval from their Boards. Here’s some examples:
