Salūd Primary Care is a new, modern medical practice formed in 2011 to offer a fresh approach for delivering healthcare to patients in the Hartford region. Our strategy is to promote optimal health and wellness through the principles of innovative, evidence-based medicine and the integration of complementary and alternative treatments.

Salūd Primary Care is a Patient-Centered Medical Home and is a member of Saint Francis HealthCare Partners.

Founder of Salūd Primary Care and board-certified in Internal Medicine, Dr. Nieves obtained his medical degree at Cornell University School of Medicine and completed his training at the Yale University Primary Care Internal Medicine Residency Program. Unique to Salud PC offerings is Direct Primary Care, a model that is based on delivering primary care services which results in better quality of care, improved doctor-patient relationships, and greater patient satisfaction
