Southern Prairie Community Care (SPCC) is collaboration between 12 counties located in southwestern Minnesota. The participating counties of SPCC are: Chippewa, Cottonwood, Jackson, Kandiyohi, Lincoln, Lyon, Nobles, Murray, Redwood, Rock, Swift and Yellow Medicine. SPCC was formed in July 2012 as a Joint Powers entity.

Our counties share similar beliefs and values and a vision to improve the quality of life for their residents. We also share higher than state average rates of poverty, obesity, physical inactivity, diabetes and other health indicators. Because of this, we have come together to improve the health and wellness of our residents. Our mission is to enhance the quality of life for our citizens through facilitating the integration of services and supports provided throughout our community.

We believe that by better aligning the services and supports that our residents receive from providers, county services (broadly defined to include public health, human services, social service, police, courts, treatment, and housing), inpatient facilities, outpatient facilities, emergency rooms, and other community organizations, we will be able improve our citizens’ quality of life and decrease the overall cost of services for government funded programs, county-provided services, and supports for the individuals that we serve.

Key Focus Areas Guiding Development of the SPCC Care Model:
•Assuring a strong primary care system that fully utilizes the State’s Patient-Centered, Health Care Homes concepts;
•Focusing our efforts where we can most likely impact the total cost of care and services, member satisfaction, and quality (IHI’s Triple Aim);
•Using data and best practices to support and provide focus to our work and confirm of our outcomes;
•Facilitating better coordination and communication between service and support providers; and
•Engaging participants in ways that are culturally appropriate and understandable to them.
