UF HealthStreet

HealthStreet is an innovative community-engagement program that aims to reduce health and research disparities by linking community members to social and medical services and opportunities to participate in research that is relevant to them.

HealthStreet’s community health workers (CHW) assess health concerns, conditions and research perceptions of community members. This occurs where they live, work, play and pray: including laundromats, libraries, bus stops, community centers, health fairs, senior centers, and churches.

CHWs provide referrals to community members for medical and social services, as well as opportunities to participate in health research, relevant to their health concerns and health conditions. HealthStreet is located in Gainesville, FL, but reaches people in North Central and North East Florida, including Duval County.
